چکیده

Up to 1934 only six of the 300 cases of dissecting aneurysm of the aorta described in the literature had been diagnosed ante mortem (Shennan, 1934). Many correctly diagnosed cases have since been reported (with decreasing pride) and the condition is now well enough recognized in general medical practice for cases to be reported only when they present some unusual or particularly instructive feature. Dissecting aneurysm has been wrongly diagnosed ante mortem as syphilitic aortic disease on many occasions and confusion can also occurpost mortem. The importance of this differential diagnosis tends to be overlooked, especially in the standard textbooks on syphilology and cardiology, and even in recent monographs on syphilitic cardiovascular disease. If the current decline in the incidence of syphilis continues, there is likely to be a relative increase in the incidence of dissecting aneurysm-a phenomenon that has already been noted (Simpson, 1947; Newcomb, 1953). A personal, salutary experience has prompted a review of the subject from this point of view. The pathogenesis, symptomatology, and diagnosis of dissecting aneurysm are briefly described first of all (giving references for further study), three illustrative cases are reported, and the differential diagnosis from syphilis is then discussed.

Epidemiological features of ruptured dissecting and saccular aortic aneurysms.

An analysis based upon necropsy figures has been made of the frequency of deaths with dissecting aneurysms and ruptured fusiform or saccular aortic aneurysms. More deaths from these conditions were found at necropsies conducted on people dying suddenly in the community than in those dying in hospital. Dissecting aneurysms were nearly all found to involve the thoracic segment of the aorta and we...

Surgical aspects of dissecting aortic aneurysms.

From the Departments of Surgery, The University of Michigan Hospital, Ann Arbor, Michigan and the Alexander Blain Hospital, Detroit, Michigan. Great progress has been made in recent years in the surgical treatment of syphilitic and arteriosclerotic aneurysms as well as other lesions of the aorta (1, 2), but very little advance has been made in the treatment of dissecting aneurysms. We have been...

Recognition and management of dissecting aneurysms of the aorta.

The current approach to treatment at St. Mary's Hospital of dissecting aortic aneurysms uses a period of induced hypotension before aortography and consideration for surgical correction. There are 13 survivors of a series of 24 patients treated in this way.

Traumatic dissecting aneurysms.

Most dissecting aneurysms of the aorta are spontaneous, and have as their underlying pathology a degeneration of the media. Strain, physical and mental, has been suggested as a factor which precipitates their onset, but external trauma is usually exonerated. Shennan (1934) traced only six cases due to trauma, and he formed the opinion that there was rarely a clear relationship between these ane...

Absence of elastica from inner half of aorta. Multiple dissecting aneurysms.

Multiple dissecting aneurysms were found in the course of an autopsy on a 52-year-old male patient who had expired suddenly as a result of a massive pericardial tamponade. Two of these aneurysms involved the aorta. The others involved muscular arteries, namely, the superior mesenteric and the splenic. In the case of the aorta, there was an absence of elastica from the entire inner half of the m...

Dissecting abdominal aortic aneurysm in Ang II-infused mice: suprarenal branch ruptures and apparent luminal dilatation.

AIMS In this work, we provide novel insight into the morphology of dissecting abdominal aortic aneurysms in angiotensin II-infused mice. We demonstrate why they exhibit a large variation in shape and, unlike their human counterparts, are located suprarenally rather than infrarenally. METHODS AND RESULTS We combined synchrotron-based, ultra-high resolution ex vivo imaging (phase contrast X-Ray...
